The visit log
What we measure, and how we keep it.
Each patient has one log. At every visit we add the same four measurements, so the new numbers sit beside the old ones. The log records what was measured. It does not predict or promise an outcome, and results vary.
We read the log with you before you leave and give you a copy of the prescription.
Every entry
What one entry holds
- 01
Eye length
How long the eye is, front to back, in millimeters. A scan that takes about a minute for each eye.
- 02
Prescription
The power in diopters, with focusing relaxed by drops at the evaluation and at the yearly visit.
- 03
Curve of the front of the eye
A map of the front surface. It guides lens fitting, especially for overnight lenses.
- 04
Eye health
The doctor looks at the front and the back of each eye and records what was seen.
- 05
Notes from home
Comfort, how long lenses or drops are used and anything the child has said about seeing.
- Sudden vision loss, flashes of light, a curtain over your vision, a chemical splash or an eye injury needs care now. Call our office right away or go to an emergency room. If you can't reach us, call 911.

A routine yearly exam is billed to a vision plan.

Questions you want to ask
About the schedule
How often are the visits?
After the evaluation, usually a short check after a month of a new option and a full set of measurements about every six months. The doctor sets the schedule with you.
Can the log be shared?
Yes. Ask for a copy for your child's other doctors or for a second opinion.
How much will this cost?
It depends on the visit and on your plan. How fees work explains which part a vision plan pays, which part medical insurance pays and what you pay yourself, and we give a written estimate in advance.
Book a myopia evaluation
The first entry in the log takes about an hour.
